By submitting this application, you acknowledge and agree to the following terms and conditions of membership with the West Valley Empowered Voices Lions Club (“the Club”), a charter-pending Lions Club under Lions Clubs International District 21-North.

1. Membership Approval
   Membership is subject to approval by the Club’s Board of Directors or its designated membership committee. Submission of an application does not guarantee acceptance.

2. Membership Types
   Applicants must select an appropriate membership type (Standard, Veteran, Student, Leo Lion, Former Member, Transfer).

• Standard applicants are individuals who are currently or formerly affiliated with any Lions Club.

• Veteran applicants are standard members who are active duty or retired military.

• Former Member applicants are former members of any Lions Club who are not currently active.

• Transfer applicants are active members of a Lions Club transferring their membership to West Valley Empowered Voices.

• Leo Lion applicants are individuals between the ages of 18 and 30 who are current or former members of a Leo or Lions Club and must provide their Club name and membership ID.

• Leo applicants are youth between the ages of 12 and 17 participating in service through the Club’s youth program.

• Cub applicants are children between the ages of 0 and 11 who participate in Club service projects under parental or guardian supervision.

3. Dues & Payments
   Annual dues are assessed from June 1 – May 31. Applications submitted mid-year will be prorated. Membership becomes active once payment is received and processed through Stripe or another approved payment platform.

4. Charter-Pending Status
   Until the Club receives its official charter from Lions Clubs International, membership is provisional. All payments collected support administrative and program development expenses required to achieve charter status.

5. Participation & Conduct
   Members are expected to participate in meetings, community projects, and fundraising activities to the best of their ability. Members agree to uphold the ethical standards, mission, and values of Lions Clubs International and the Club’s local code of conduct.

6. Privacy & Data Protection
   The Club respects member privacy. Personal information collected will be stored securely and used only for internal Club operations, reporting, and communications as required by Lions Clubs International.

7. Cancellations & Refunds
   Membership dues and donations are non-refundable once processed, except in cases of administrative error. Members may withdraw from the Club at any time by written notice to the Club Secretary.

8. Liability Disclaimer
   Participation in Club events or volunteer activities is voluntary. The Club and its officers are not responsible for personal injury, loss, or damage sustained during official or unofficial Club activities.

9. Amendments
   These terms may be revised by the Club’s leadership as needed to maintain compliance with Lions Clubs International policies or local legal requirements.
